Would that be to fix it to the substrate only or does it include the surface preparation prior to tiling?

I'd include priming in that price. I suppose each job is a bit different, the price may go up if there is a lot of cutting to do, I certainly wouldn't go any higher than £6 per metre. From experience I've found that I can comfortably prime and fit 40m + in a day, and that provides a nice wage at a fiver per metre.
