Hmmm, a difficult one, which lug is it? If it is the one that is able to move up and down a bit to keep the face plate level, then these can work loose and fall out, if you still have it (or remove one from a new box) then you can pry up the two retaining pins that it slides on and slip it under again.
If it is the fixed lug on the other side, then I'm not sure how to proceed.
