What is the most popular reasons why tiles pop off walls?

Can be a number of reasons...

1: one can be expansion issues and the tiles will tent off the wall due to lack of perimeter joints or butt jointing tiles together.

2: lack of contact with the adhesive bed can cause tiles to de-bond under stress thermal or moisture.

3: tiles too heavy for the substrate.

loads of reasons really, some more info would be great..
