I do Finnish wet rooms all the time, and the fall rate is at least 20mm in1mtr, more around the drain. Normally the concrete subfloor is roughly levelled by the concrete pouring company, but I always screed it myself anyway. even then its not great, and before tiling, I use tile cement adhesive to acurately straighten any dips, and esp next to walls. This is of course, after primering, and then waterproofing and taping all the 'joints'. Here we use a product called Kerafibre, and needs .5mm thickness on the floors, .4mm walls. and taped and painted with wtrprfng around shower pipes and all board floor wall joins. I have pictures if anyone is interested.
