I checked with a right angle tool this morning - everything is 90 degrees and square.
Nothing is out. I had a complete dream of the whole trim issue last night!!!
I think it is the way the tile trims are fixed - they are not symmetrical - cos we are trying to match one trim to another and then at the corner - three trims come together and if the row of trims from window to side of cladding and then down side length of cladding are in line - that's OK - but if one length of trim is put in the other way - then they don't line up...does that sound logical to you?

If everything is at 90 degrees I can only think that the tile on the flat surface is cut at the wrong angle. It appears to run out of square in the 2nd photo. The trim should have been fitted first and the tile cut and fitted to suit the angle and corner required.

Gentlemen,

The corner trim has been installed - thanks to all for your invaluable input, I decided to ask the tiler to turn the corner trim downwards and it worked!

Here is the finished photo. Looks beautiful and absolutely ticks all the health and safety boxes for me.
