Screed is usually very brittle, so once it has cracked the adhesion to tiles usually begins to fail.
It is a hard process to remove screed so avoid this wherever possible.
As the screed is breaking up and most possibly not fixed to the pugging underneath, I would advise you to use an acrylic based resin to infuse the crack(s). You can also use resin to treat the edges of exposed tiles, which will help consolidate it. Large cracks and gaps in the screed will usually need an epoxy mortar repair.
