kitchen floor 2 substrates

Hi All,
Just been to measure up for tiling a kitchen floor through to utility and cloakroom. all in all about 23m2.
problem is that the utility, cloak and half the kitchen are concrete floor really uneven and sloping away from the other half of kitchen floor which is t&g floor boards. house is very old but floorboards are really solid with no noticable movement.
question is what is best method to
a; level all surfaces
and b; tile onto both substrates and with what adhesive
customer has indicated tiles will be either ceramic or porcelain.

you will need to overboard the t&g so maybe using 6mm cement boards would be the easiest way to go about this,levelling it all!
would use flexible adhesive and grout and would use an expansion joint between both substrates

if i use 6mm cement boards on the t&g then it will be over 25-30mm lower at the worst areas of the concrete floor
flexi ad and grout wont fill this out will they?

mapei fibreplan self levelling on the concrete will bump it up 30mm in one go,that should level it all
