Looks like the water damage didn't help, but IMO flexible rapidset on it's own isn't suitable for that particular job. If tiling straight onto 18mm ply on the joists, bare minimum spec for me would be rubber crumb throughout and all joints taped. Had a few small, seemingly deflection free floors give problems in the past (tiles over joins popping) so now always overboard or use rubber crumb.
 
I've now started using Bal fast flex on all my wooden floor jobs since I noticed some hair line cracks in grout line in a floor I tiled Not long ago. Also I stopped using ply a couple years ago now.
 
well on my last visit the customer was happy for me to redo the tiling ,after floor strengthening,but i told him no thanks.......once bitten
i told him id refund money too.
today i went to rip it all up and i found a water damaged area near to where the first cracked grout showed itself
looks like a fair bit of water sat there for a while,but they have told me absoluotly no washing machine or waste pipe leaks,which i find hard to swallow

so im not happy to refund all the money,ill contact him tonight
if you dont tile the whole floor only where the washing machine goes if this leaks it fills the void between ever side before it shows on the floor if whole floor tiled it comes out at your feet and can be delt with there and then not stay hidden making its way under the floor
 
Looking at the images the adhesive has come clean away from the ply,am I right in understanding that Topps Ady is made by Palace,I stopped using Palace for that reason even after priming using SBR this happened to me .Then again how long do you guarantee your work for ?, me personally its only 12 months I always tell the customer first if they have done all the prep work,such as the laying of the ply.
