Everyone creates a Health & Safety Policy that is unique and tailored to meet the needs of their own business.

There is no real set in stone template to follow.

Just outline aspects of your business and paperwork that is suited to each area e.g. risk assesments, coshh and applicable fire assesments and RIDDOR etc.

It all depends on your own circumstances, a business with premises would be more detailed than a sole trader who uses a dommestic property for admin etc.
