I have done a few of these types of formers on shower room blocks in the 70s while working for the MOD on a few barracks.Back then all the floors were done with either pit sand or sharp sand you would lay the screed and tile,if you laid 10 sq yd that was a good days work.

The D

On this one the floor needed lifting 100mm for wheelchair access so I screeded the floor in S&C and dropped a former in for the shower area.
