Advice how to cut out a square hole.

depends on the type of tile, ceramic or porcelain.
if it's porcelain, lately I do not make holes in the corners,
cut directly on the front and round the corners with a finger bits.
thus I avoid breaking the tile during installation and even after.

Should the tile cut be against the trim or the wall?
Also, are corners outside wet areas just grouted rather than Silicon?

What trim? you asked about cutting a square hole in the middle
of a tile
 
Are you talking about in a window reveal? Cut edge against the window. Good edge against the trim. Same with niches and tiles that happen to be near another wall when the other end of the tile is against the trim. Clear as mud right ?! 😀
 
I think we need to know where he is fitting this trim before giving
advice, nobody has mentioned a window or niche
 
I’m asking where he is putting it and answering where it might be going.

As we all know, there are also many situations where you would have a cut edge against the trim.
