There's not a lot that can be done now...only time will tell but you may have got away with it this time.

If you get a crunching sound when walking on them or some become loose then you will be looking at problems.

That looks like a flaw in the tile.
As already stated ufh should have been commissioned & it's too late now to worry about it.

Also one more question - does this only need to be done when it is first installed, or would I need to do it after a period of non-use (I probably won't use it over the summer for example)

To be safe, after any long period of time where the UFH hasn't been used you should turn the heating on over a period of a few days to prevent any kind of thermal shock.
