Moisture & Efflorescence Coming Through Grout

Could the old quarries have been used as a rudimentary DPM? So by lifting some and then patching this has allowed passage of moisture from below. If you're back to leveller I'd be going with an epoxy DPM. Ultra do a two pack, colour coded, then Prime it N then your good to tile. Should only raise the level by a couple of mm.

I guess moisture could be getting through from the areas of the sand & cement I took out......but again it was dry under these areas of the screed but no dpm!
 
I've just done a similar thing with a vinyl wet room. The original floor was laid about 18 months ago (Not by me)
The latex had blown under it and after ripping up the vinyl I found it to be really damp. The bungalow it's in was originally a doctors surgery any there are terrazzo tiles fixed throughout with no DPM. I used Ardex NA, then Ardex DPM1C, NA over the top then fitted new vinyl. I've used their liquid DPM a few times and never had any problems after.

Definitely a puzzle this one mate! I'm not an Ardex user so can't comment on their products but I assume the process is similar to Ultra. Basically sandwich an epoxy DPM between latex levellers and tile on top. I know if height is an issue you can use their neoprene primer direct onto the DPM then tile. That's the process I use for vinyl, you'd have to check the spec of the dpm to see if tile adhesive can be applied direct to it or if it needs a coat of NA first. I'm not familiar with TM products either so you'd have to check if it's suitable for use under a dpm.

If a burried copper pipe feeding that rad is pin holed.....

You say a plumber has been out? Is the heating pressurised or is it gravity fed?

When they had plumber out they said he checked pressure and all was as it should be so I'm guessing pressurised......
 
It seems you lifted the tiles that looked worse first and they are around the rad, if you find a leak it will look better on you.
Seen tiles on a kitchen floor crack and sink, that was down to a pin holed copper pipe.
